Health Care Organizations

Increasingly healthcare is being held accountable for patient health outcomes – and despite the newest treatments and best professionals – health often comes down to how well patients and families can do their parts. CCL members are pros at working on the social and behavioral underpinnings of health. Older Adults and their Families

Taking good care of yourself and your loved ones as they age can be tough. Fortunately, New York City has some of the best resources in the nation – from expert health professionals in geriatric medicine (AGS), to health plans for older adults (Medicare.gov); to social services (DFTA). A one-stop place to get guidance is New York Connects – the resource center for older adults and their families looking for referrals. Moreover, while we expect that our main customers will be the “third party” payers of health and long-term care (e.g., health plans), the may be some services that consumers want to buy out of pocket.
