Selfhelp Community Services is a not-for-profit organization dedicated to maintaining the independence and dignity of seniors and at-risk populations through a spectrum of housing, home health care and social services, and leads the field in applying new methods and technologies to address changing needs of its community. Selfhelp continues to serve as the “last surviving relative” to its historic constituency, victims of Nazi persecution.
Located in 27 sites across Manhattan, Brooklyn, Queens, the Bronx, and Long Island, Selfhelp serves more than 20,000 clients each year. In addition to its housing and home care programs, Selfhelp operates senior centers, NORCs, case management services, Alzheimer resource programs, and NY Connects in Queens. Selfhelp offers falls prevention programs (MOB, SAIL, and Tai Chi for Arthritis), and also has experience providing CDSMP (for arthritis and diabetes, and in English and Chinese), Walk with Ease, and Care Transitions.
